Public Health Passenger Locator Form

It is a requirement of the Irish Government and the Irish Department of Health that all passengers arriving into Ireland fill in a Public Health Passenger Locator Form. This includes Irish citizens coming home. This form can be downloaded here.

If you haven’t completed this form on the aircraft, please:

  • Complete and return the Public Health Passenger Locator Form before you clear Passport Control or enter the Arrivals Hall.
  • Return your completed Public Health Passenger Locator Form to an Immigration Officer.

You may be contacted during the 14 days after you arrive in Ireland to check that you are self-isolating.
